Appointments and Cancellations

Please note that all sessions are conducted via TeleMed and are by appointment only. To avoid disrupting the appointment that follows yours, it is important to be on time. Each session lasts 50 minutes and begins within 10 minutes on top of the hour.

If you schedule an appointment, we kindly ask that you make every effort to keep it, even if you feel upset about the previous session or anxious about the next one. If you know that you won’t be able to attend, please leave a confidential voicemail message as soon as possible. The confidential voicemail is available 24/7. 

Appointments must be canceled at least one week in advance. Late cancellations and no-shows will be charged for at least half of the session. In case of an emergency and you are unable to reach our office, please call the crisis line at 838-4428.

Fees and Billing

The initial session is $245, while each subsequent session requires a payment of $210 per hour. All payments are processed on a monthly basis through autopay, utilizing either a credit or debit card, upon completion of insurance processing.

Client Responsibility

Before seeking individual outpatient mental health treatment, it’s crucial to contact your insurance provider and confirm your coverage. It’s your responsibility to ensure that your account is paid in full. If necessary, you are responsible for any and all collection fees, at which time client confidentiality will be waived.


Insurance

Dr. DeSeve will handle billing for your primary insurance provider, while it is the client’s responsibility to bill a secondary provider. Please keep in mind that many insurance agreements mandate that clients authorize their therapist to provide a clinical diagnosis and a treatment care plan or summary.

Your First Session

Before your first session, please download, fill out, and submit the following three forms: Registration; Privacy Practices, Financial Policies, and Consent to Treat (Washington Notice of Privacy Practices); and Bio-Psychosocial History. Scan or photograph the Registration and Privacy Practices forms and send them via text or email and mail the Bio-Psychosocial History form. Contact us if you have any questions.
